phantom-hooks-referenced-in-docs-not-in-settings
Hooks like startup-routine-gate and brainstorming-gate were referenced in MEMORY.md and master-orchestrator SKILL.md but had zero entries in settings.json and no files on disk. These phantom references create false enforcement claims — any hook cited in documentation must be verified to exist in the hooks registry. Audit with grep across settings.json before trusting enforcement claims.
