MEMORY.md Tier 1 Directive 0 and master-orchestrator SKILL.md both referenced startup-routine-gate and brainstorming-gate hooks that never existed in settings.json or on disk. Zero runtime enforcement despite strong documentation claims. Any hook name appearing in documentation must be verified against settings.json registered hooks; documentation and runtime state must match exactly.
