MEMORY.md has a hard 200-line truncation limit in Claude Code’s context loading. When the file approached 190 lines, the Topic Files section (critical pointers to all topic files) was only ~15 lines from the cliff, meaning one more addition would silently drop those pointers. The fix is to keep MEMORY.md under 160 lines as a safe buffer, extracting all inventory/detail to topic files.
