Claude Code truncates MEMORY.md at ~200 lines silently — content beyond that limit disappears without warning. At 190 lines, the Topic Files section (the pointers to all topic files) was 10 lines from falling off the cliff and would have silently vanished. Refactor triggered: 190→126 lines by extracting inventory (hooks, tool-intelligence) to topic files, leaving only directional pointers in MEMORY.md.
