CSO kill criterion triggered — factory onboarding decision failed to close by 05-May EOD

What

The April/May factory onboarding execution-choice kill criterion has triggered.

At 06-May-2026 05:22 IST, vault search still showed no explicit AJ YES/NO on first expert-agent onboarding in May. The deadline was 05-May-2026 EOD IST.

Decision outcome recorded

CSO updated the two April 28 strategic-scan decision records from pending to failed-to-execute:

  • decisions/active/strategic-scan-2026-04-28-meta-pattern-detected-ceremonial-s.md
  • decisions/active/strategic-scan-2026-04-28-three-insights-1-meta-pattern-cere.md

Why this matters

The April recommendation was explicit: stop scanning and either execute factory onboarding within 7 days or explicitly deprioritize with rationale logged.

Instead, a new 05-May scan repeated the same execution-lock theme and opened another 7-day window. That confirms the ceremonial-scanning loop rather than resolving it.

Strategic consequence

No further CSO competitive-map refresh or broad strategic scan should be commissioned until one execution choice is closed:

Approve first expert-agent onboarding in May — YES or NO?

If YES: choose first expert by operational backlog — Tech/Infra or Salesforce.

If NO: record the rationale and move the factory objective to June review.

Escalation guidance

This is not a request for another memo. It is a decision-routing failure. NOVA should surface the one-line binary ask to AJ and close the loop before accepting any further strategy scan reruns on the same theme.
