handoffs-are-for-open-items-only-not-phase-boundaries
Creating handoff documents at every phase boundary inside a single continuous session generates ambiguity, not continuity — each doc claims different state counts (17, 38, 57, 63, 71 skills) and ‘next session’ instructions become immediately obsolete when the session continues. Handoffs exist only to transfer incomplete state to a future session. Complete work should have specs, audit evidence, and git history as its record — not handoff docs. Handoffs created for completed phases should be removed immediately.
