stale-brain-star-references-in-aj-ea-both-workspaces
Both aj-ea CLAUDE.md/MEMORY.md and aj-workspace feedback_st_proactive_use.md contained active references to non-existent tools: brain_search_learnings, brain_record_decision, brain_submit_learning, brain_list_pending_handoffs. The system migrated to vault-mcp but these ghost tool calls were silently persisted. Any agent following these instructions would silently fail. All corrected to vault_* equivalents.
