Both aj-ea CLAUDE.md and both workspaces’ feedback_st_proactive_use.md contained active references to brain_record_decision, brain_submit_learning, brain_search_learnings, brain_list_pending_handoffs — tools that do not exist. The system uses vault-mcp exclusively. These stale references persisted silently across many sessions as Law 2 (Zero Ambiguity) violations.
