Template→live file propagation gap: When upgrading orchestrator/specialist templates (templates/orchestrator-template.md, templates/specialist-template.md), the improvements are NOT automatically propagated to existing live agent instruction files (architect.md, chief-of-staff.md, explorer.md). MUST manually propagate all template changes to all live files in the same operation. Quality audit caught 5 defects where template had goal_id, [SKILL: x] markers, and goal constraints but none of the 3 live orchestrator files did. Add to deployment checklist: template change → grep all live files → propagate.
