CCVS deep-integration sweep across 8 Helix orchestration files complet…

Decision

CCVS deep-integration sweep across 8 Helix orchestration files complete: 4 orchestrator skills (quality/delivery/coherence/master) + 4 memory docs (protocols.md Phase 4 explicit gate, pristine-sweep-protocol complementary half, plugin-integration L6/L7 wiring rows, specialized-protocols ADR section). Each integration is focused single-paragraph + cross-link to authoritative codex-cross-model-verification.md doc. Pristine Sweep on all 8 files passes 7/7 (link integrity, no duplicates, cross-refs resolve, doc budget, /tmp clean, JSON valid, codex auth live).

Rationale

Honest first-principles decomposition: 4 of 8 edits (orchestrator skills) genuinely change agent behavior when those layers run — quality-orchestrator now dispatches CCVS as parallel reviewer at Stage 5 GATE, delivery-orchestrator’s Stage 1 PLAN expects CCVS HARD_GATE at Stage 2 EXECUTE, coherence-orchestrator escalates to CCVS on hard_gate-path drift, master-orchestrator halts AUTONOMOUS mode on missing/BLOCK CCVS verdict. 4 of 8 edits (memory docs) are reinforcement/discoverability — they help agents find CCVS at decision points (Phase 4, ADR formation, pristine sweep, L6/L7 wiring lookup) but don’t add NEW behavior beyond what the autonomous hooks already do. Not theatre because: hooks are the autonomous ground truth, docs reinforce at human-readable layer where Claude reads them at decision points. Confidence 0.92 not 1.0 because: (a) risk of doc drift if codex-config.json changes (8 places to update), (b) some integrations dormant in SURGICAL/Quick-Flow paths that skip orchestrators, (c) end-to-end AUTONOMOUS-mode validation not yet performed. Supersedes prior decisions 8795002f (initial 0.85) and af39cca7 (post-CLI-fix 0.95) — adds the Helix-deep-weave dimension.
