The Stop hook’s ‘pure Q&A/research/exploration/planning with no file modifications’ exemption clause does NOT apply when the brainstorming skill’s hard-gate is active. The brainstorming skill’s terminal state requirement (must reach writing-plans invocation) explicitly overrides the standard exemption. When the agent is mid-brainstorm awaiting user input, repeated Stop hook blocks are expected behavior — the correct response is to continue waiting for user input rather than treating each block as an error condition.
