The [REFERENCE] tier in session-learnings.md was removed Apr-2026. Only [CRITICAL] and [OPERATIONAL] remain. The rationale: REFERENCE entries are look-up-able from first principles or docs in under 2 minutes and do not change future session behavior. Any future candidate entry attempting [REFERENCE] classification should be discarded — if it passes the behavioral intake gate it is [OPERATIONAL], otherwise it does not belong in session-learnings at all.
