The stop hook repeatedly approved stopping (no file changes) while the brainstorming skill’s hard-gate explicitly forbids it before design presentation and writing-plans invocation. Sessions that start brainstorming must be treated as ‘in-progress workflow’ by the stop hook even when no files are modified, not as ‘pure Q&A’.
