After 5 consecutive stale runs (no improvement beyond confidence margin), further mutation of the existing prompt rarely breaks the plateau. The correct response is full prompt regeneration from first principles, not continued mutation. autoresearch v2.5.0 encodes this as the Plateau Breaker rule in the Strategy Guide section.
