mcp-tool-namespace-discipline-fully-qualified-format-required
MCP tool references must use Server:tool_name fully-qualified format whenever two or more active servers share a tool name. The model silently picks one server when names collide — there is no error, warning, or disambiguation prompt. This produces outputs that appear correct but are effectively random between the colliding servers.
